You can now request an appointment anytime, day or night. Just complete an online appointment request form.

All new patients can now complete pre-registration forms and current patients can update information with just a click of the mouse.

4. Complete your annual exam health history forms for your annual exam, a symptom assessment for your problem visit, or your hormone evaluation online prior to your visit.
5. All information will be transmitted to our office, no need to print anything

Request your refills online, and we can approve and send an efax to your pharmacy on file. Refills were never faster or easier!

3. Complete all information concerning the medication you would like refilled
4. Choose your pharmacy from the drop down box, or provide pharmacy name and fax information
5. We will email you after your request has been completed

If you have a minor problem, and would like a consultation without coming to the office, have your visit ONLINE! Perfect for those appointments that don't require an exam, answer a thorough set of questions online, when it's convenient for you, and we can respond to you online! No traffic....No waiting rooms....

3. Request your provider and symptoms you are having
4. Enter your credit card information. Your credit card will only be charged when we complete the virtual office visit
5. Complete all information concerning your symptoms.
6. Your provider will see your virtual office visit request, email you if we need more information, or email you to complete your virtual office visit. Your prescription, if necessary, will be efaxed to your pharmacy.

** To have a virtual office visit you must be an established patient of our office, and have a current physical exam within the past year. We will not prescribe narcotics or controlled substances through a virtual office visit. Virtual Office Visits are only done during office hours, so that we will have access to your medical records. All virtual office visits are printed and stored in your medical record. Virtual Office Visits are for mild, follow up conditions, and not appropriate for emergency or severe conditions. Virtual Office Visits are optional, you may always call our office for an appointment, or be seen in the closest emergency room for emergent issues. 

With enhanced messaging, you can now receive test results or messages from our office whenever it is convenient for you. Once you are enrolled, we can use our portal to send you secure email messages. Whether it's appointment reminders or office closures, enhanced messaging allows us to contact you via our secure portal.
