Let's take the "wait" out of our waiting
room! Effective June 1, 2008, we will require all paperwork for your appointment
to be received prior to your appointment.
Paperwork can be completed online by logging in to
our online services, or downloaded, printed and faxed to 817-488-2820 24
hours prior to your appointment.
Registration Forms
In an effort to decrease patient wait
times, we offer two different ways to complete
your registration and insurance information PRIOR
to your appointment.

You may log on to our online
services here, and complete
all your registration forms online. No need to print anything, all
information is transmitted to our office via our HIPAA compliant
online portal. Now you are ready for your office visit.

You may download and print all forms
below and fax to 817-488-2820 at least 24 hours prior to your office visit.
Failing to
complete your forms prior to your appointment will cause your
appointment to be delayed.
